Video Mix Lab, Inc. In the PrivacyMark system, the Japan Institute for Promotion of Digital Economy and Community (JIPDEC)Japan Institute for Promotion of Digital Economy and Community (JIPDEC) or a designated organization examines whether a business's personal information protection system complies with JISQ15001 (Personal Information Protection Management System - Requirements) for the certification to be granted by JIPDEC. The PrivacyMark system was established in April 1998 by JIPDEC. We acquired the certification in January 2019 and are working to protect personal information.

You can request "notification of the purpose of use, disclosure, correction, addition, or deletion of content, suspension of use, deletion, and suspension of provision to third parties" (disclosure, etc.) of your personal data.

(2) To confirm your identity, please send or bring one of the following documents together with "Prescribed Form".

・A copy of a public certificate such as a driver's license or passport that can be used to identify the person making the request for disclosure.
・A copy of the certificate of residence (made within 30 days prior to the date of the request for disclosure.)

(3) If the request is to be made by an agent, in addition to (1) "prescribed form" and (2) "identification documents," the agent must also send or bring one of the documents listed in (a) and (b) below.

(a) Identity verification documents of the agent

・A document proving the identity of the agent, a copy of the agent's driver's license, passport, or other official document that can confirm the agent's identity. (A copy of the name and address of the agent making the request for disclosure, etc.)
・A copy of the certificate of residence of the agent (made within 30 days prior to the date of the request for disclosure, etc.)
・If the agent is a lawyer, a document showing his/her registration number.

Only in the case of a request for notification or disclosure of the purpose of use, the following amount (including the cost of our reply) will be charged per request. Please enclose a postal fixed-amount money order for the following amount when mailing the request documents.

3. Notification of review results

 We will notify you of the results of our review without delay by the following methods.

・In the case of notification or disclosure of the purpose of use of personal information, a written document will be mailed or hand-delivered.
・In the case of correction, addition, deletion, suspension of use, deletion, or suspension of provision to a third party of personal information, we will contact you by phone or e-mail.

One of the following correspondence methods will be conducted at our discretion.
・Sending the response by mail to the registered address of the individual.
・Emailing the response to the registered email address of the person.
・Calling the registered person's telephone number and reply orally.
